Key Responsibilities
Support the behavioral health needs of patients by delivering
evidence-based treatment for mental health issues
Collaborate with care teams to manage psychotropic medication and
ensure effective treatment adherence
Document patient assessments and facilitate treatment plan
adjustments as needed
Required Qualifications
Master's degree in Social Work
Current California independent licensure required
2+ years of prior healthcare/behavioral health experience
required
Geriatric care experience highly desired
Proficient in computer skills and electronic medical record (EMR)
documentation
