Job Description:
A company is looking for a Care Manager RN.
Key Responsibilities
Develop and facilitate complex care management activities for
members with physical needs
Collaborate with providers and community resources to address
members' unmet needs
Monitor member status and revise care plans as necessary to meet
goals
Required Qualifications
Degree from an Accredited School of Nursing or a Bachelor's degree
in Nursing
2 - 4 years of related experience in case management, home health,
or managed care
Registered Nurse (RN) state licensure or compact state licensure
required
Experience in developing personalized care plans and education for
members
Ability to provide resource support for local services based on
assessments
