Job Description:
A company is looking for a Clinical Program Manager, Specialty
Population Health.
Key Responsibilities
Lead a team of Care Extenders to implement and enhance the
Adolescent and Young Adult Suicide Prevention Program
Develop and coordinate community resource guides and ensure
comprehensive service plans for members
Monitor team performance, provide training, and ensure compliance
with program goals and Key Performance Indicators
Required Qualifications
Clinical licensure (LCSW, LPC, LMFT, or equivalent) is required
3-5 years of experience managing a team and implementing
programs
Master's degree in behavioral health, social work, or healthcare
management from an accredited university
Experience in management and compliance with laws and
regulations
Ability to develop and maintain relationships with internal and
external partners
