Position Overview:



Reporting directly to the Unit Supervisor and working under general supervision, the Triage -Specialist is responsible for assisting individuals in determining their eligibility for Medicaid programs and services. This role involves intake, screening, and triage, ensuring that individuals receive the appropriate services to meet both their immediate and long-term needs. The specialist will also collaborate with state agencies, healthcare providers, and other professionals to facilitate Medicaid eligibility determination and enrollment.


Key Responsibilities:

Medicaid Eligibility & Application Assistance


  • Assist individuals in determining eligibility for Medicaid benefits and refer them to the appropriate Department of Children and Families (DCF) / Automated Community Connection to Economic Self-Sufficiency (ACCESS) staff for application.
  • Coordinate with DCF and CARES (Comprehensive Assessment and Review for Long-Term Care Services) staff to facilitate eligibility determination for Medicaid Waivers.
  • Assist individuals in obtaining necessary Physician Referral Forms (3008) for Medicaid Waiver applicants.
  • Track and monitor Medicaid applications throughout the eligibility process.
  • Input Medicaid Waiver Timeline data into the Enrollment Management System (EMS) and eCIRTS.
  • Conduct follow-ups with individuals on the Assessed Prioritization Candidate List (APCL) to update information and screen for Medicaid eligibility.
  • Act as an advocate for consumers, working with CARES and DCF/ACCESS staff to resolve eligibility issues in a timely manner.


    Community Outreach & Coordination


    • Standardize and enhance outreach efforts to increase public awareness of Medicaid programs and services.
    • Develop, compile, and distribute materials to educate individuals on Medicaid benefits and access points.
    • Build relationships with service providers, hospital discharge planners, and nursing home social workers to facilitate referrals and increase awareness of Medicaid resources.
    • Conduct training sessions for ADRC access points to ensure they are informed about Medicaid services and eligibility processes.


      Administrative & Disaster Response Support


      • Maintain accurate Medicaid eligibility tracking data to ensure completeness, accuracy, and timeliness.
      • Participate in meetings with DCF, CARES, and other relevant entities to improve the Medicaid eligibility process.



        Qualifications & Skills:

        Education & Experience


        • Bachelor's degree in a human services-related field from an accredited institution; OR
        • Associate degree in a human services-related field with at least 2+ -years of relevant experience; OR
        • High school diploma/GED with 4+ -years of experience in case management, intake, or related work in a long-term care setting.


          Skills & Competencies


          • Strong communication and organizational skills, with attention to detail.
          • Ability to utilize crisis intervention techniques to de-escalate verbal confrontations.
          • Ability to work effectively with older adults and the general public, both over the phone and in person.
          • Proficiency in Microsoft Office Suite (Word, Excel, Outlook).
          • Ability to type at least 45 words per minute with strong English grammar and spelling skills.
          • Ability to handle multiple tasks independently and within a team environment.
          • Bilingual (English/Spanish) preferred.
