Job Summary:

The Family Caregiver Support Specialist is responsible for providing caregiver support, care coordination, and public education services to the local community. This position involves assisting caregivers by offering information, referrals, and emotional support while promoting awareness of available programs and services. Additionally, the specialist will participate in community outreach, presentations, and training sessions to educate professionals and the public about long-term care options and available resources.

Key Responsibilities:


  • Provide information, referrals, and support to caregivers, family members, and individuals seeking assistance.
  • Conduct assessments, identify available service options, and assist caregivers via phone, office visits, or home visits.
  • Offer emotional support and advocacy for caregivers, ensuring they receive necessary services.
  • Coordinate service plans, monitor caregiver status, and link individuals to appropriate community resources.
  • Conduct public education and outreach activities, including presentations, speaking engagements, and distribution of informational materials.
  • Engage in crisis intervention and offer in-person assistance to caregivers in emergency situations.
  • Maintain accurate records and prepare reports related to caregiver support services.
  • Participate in workshops, seminars, and training sessions to stay informed about new services and programs.
  • Support disaster or emergency response efforts as needed.


    Qualifications:


    • Bachelor's degree in Social Work, Counseling, or a related human services field (Licensure in Social Work preferred).
    • At least five (5) years of experience in human services, preferably in crisis intervention or caregiver support.
    • Experience in customer relations, case management, or service coordination is preferred.
    • Strong understanding of community resources, eligibility criteria, and application procedures for services.
    • Excellent verbal, written, and interpersonal communication skills.
    • Ability to interact effectively with caregivers, families, and individuals in distress.
    • Proficiency in Microsoft Office (Word, Excel, Outlook) and internet-based applications.
    • Bilingual (English and Spanish) preferred.
    • Ability to handle sensitive situations with compassion, professionalism, and discretion.
    • Strong organizational skills, attention to detail, and ability to work both independently and within a team.
