Job Description:
A company is looking for a Care Management Support Coordinator
II to support administrative care management activities
remotely.
Key Responsibilities
Perform outreach to members and assist with care plan next steps
and resource connections
Serve as the front-line support for member and provider inquiries,
explaining care plan procedures
Document and maintain non-clinical member records in compliance
with state and regulatory requirements
Required Qualifications
High School diploma or GED
1 - 2 years of related experience
Knowledge of health plan activities and resources
Experience with member onboarding and administrative duties
Understanding of Social Determinants of Health (SDOH) needs
